Let us assume that the structure is having Isolated footing and cost is not the factor. How to approach this problem? What will be the type of investigation the professional agency is going to carry out ? Who will be the best agency to carry out this job? Whether this type of study has been carried out in the past? Whether any published material available on this subject?

The main worry is of Columns and Foundations?
The building has been used for What? At least are you sure that It has been designed for the that Imposed load?
Let us say The Building has been designed with for imposed load of 4 kN/m2 in the First Floor, where a Shop is run?, and so on....
Probably if you change the usage of the buiding from an imposed load of 4 kN/m2 to 2 kN/m2....( Not just the promise from the client). and remove the walls to reduce the wts...or change the Solid walls to Low wt Partition walls....on all existing floors... you may be able to maintain the loading on the foundation, with an additional floor of low Imposed Load? You may have to check all the Column now...
These are all from the legal point of view.. to prove that what you are doing is right. But the uncertainities like the quality of construction etc. are always a question mark. Probably the Building may just be standing G+3 not by design but by grace of God... and you are trying to extend that by One more floor by Proving it to others.
Best way is to leave it as it is , if you don't have any previous documents like soil reports, Structural Drawings of all the parts and the Concrete Test reports. Some times, even if you are able to convince your brain with the Codes and Standards, you may fail in convincing your heart with all your deeds , past and present.

Dear Mr Mallick
I am really sorry for such a delayed response but I seemed to have missed ur email.Once you are aware that the structure has isolated footings, and that they are inadequate and need strengthening, the approach is to open them and add to their size and reinforcements by using rebars.This has to be done very carefully and preferably by a specialised agency, and taking each foundation one by one. I am aware of this having been done by SP Repcon and KLA Constructions and further details can be given to you if required.I am aware of a handbook published by CPWD and written by Sh A K Sharma.Maybe others would like to add to the list.

There are fine suggestion given by sefians including one given by Ms. Sangeeta ji. I have still one point to doubt that even the structural designs are not available but u have not categorized the type of structure and nature of structure except that It is G+3. Further from the data available, it is clearly that at the time of construction in eighties the old code of Practice IS 456-1978 or even older could have been in use, less stress could have been on ductile detailing. the mix used could have been 1:2
:4 or even less than that and mild steel was in use so that there are may problems that u are not mentioning and we all are answering or trying to answer the question that is incomplete itself. I think it is good to do the things which are challenging but we must know that we should step out only when we know that we have a stable ground below our feet. Try safe structures

I think it is possible to extend it. Of course, it will require a detailed evaluation of existing strength--both using on site tests and sophisticated non-linear analysis such as stage construction & pushover analysis. Also remember a seismic retrofit may be required as per 1893. Sometimes all drawings are not available, and that's where experience comes in. Prudence needs to be used as to where elaborate testing is required and where its not. Such projects must be handled carefully by structural engineers experienced retrofitting & rehabilitation.
