Dear ER.  konathala kiran,
If  you have designed  this Shear Wall per ACI 318-14 or IS 456 Code , that's all you can do .  Cracks in Shear Wall  , may not be only due to Design Problems , could be due to quality of construction also .  In USA we go to a Forensic  Engineering Consultant  to find out real cause of this failure . This Forensic Engineering is a specialty and can not be handled in SEFI type of Forum .  No quick answers .

Give full Photo of this Shear wall with Cracks and other related GFC  Structural Drawings , DBR ( Design Basis Report or Structural Design Criteria ) and after that may be I can say more .
Regards.
